Parental Consent For children under 18 included in the visa application, parents not applying for the visa must complete Form 1229. If sole custody, provide court orders.
Health Insurance Proof of Overseas Student Health Cover (OSHC) for yourself and any dependents, including the provider’s name, policy dates, and policy number, if applicable.
